I really need replies to this thread please?
I have returned back to work this Tuesday after having two weeks holiday. I am quite stressed and surprised by the amount of paperwork I need to catch up on. Especially as most of it requires a deadline.
The new C.P.N. has requested for all us staff to complete one 18 pages Support Plan and another 11 pages assessment form with our key clients. What makes it difficult is I have 2 key clients whom I key work. I need to ensure all forms are completed by next Thursday. By the time frame, I am doing most of the forms at home because time at work is spent on activities etc.
The thing is, some questions on all the forms require opinions and comments from both my key clients. One key client it will be easy. But my second client it's extremely difficult to complete these forms with her.
She hates any kind of paperwork especially long ones. You can't sit down with her and go through it with her because long forms creat anxiety for her. So I can explain to her that I can fill it out on the behalf of her but require some of her input from her.
Now this is where the tricky part comes in. By the end of the 18 page form, it requires the client's signature. My key client is extremely picky on what she signs. She usually likes to read what the document is before signing it. She isn't going to stand by and listen to what the 18 pages read. In the past, she has always ripped up long documents and gets stressed.
What do I do if she refuses to sign the form? This is a very important document as it's an assessment on whether she is entitled to support etc from the council?
It's a little stressing for me as I'm not sure if the CPN is going to be happy for a non signature? Especially as the form is supposed to include the client.
I'm in a dilemma. Any advice would be greatly appreciated!
